چکیده
conclusions in selected recipients with psc, dd reconstruction is a safe and efficacious technique, with long-term clinical outcomes comparable to ryh. background while roux-en-y hepaticojejunostomy (ryh) is the common anastomotic technique for liver transplantation (lt) in patients with primary sclerosing cholangitis (psc), duct-to-duct (dd) reconstruction may be used if the recipient common bile duct is normal. there are conflicting observational data on the rate of success of dd reconstruction versus ryh, in psc. patients and methods all adult patients, who underwent primary lt for psc between 1990 and 2012, were evaluated, according to type of biliary reconstruction. recipient and graft survival, postoperative medical and surgical complications, and postoperative resource utilization rates were compared between the two groups. results totally, 73 patients fulfilled the inclusion criteria. of them, 58 had ryh and 15 had dd reconstruction. a total of 53 subjects (73%) were male, with the mean age ± standard deviation at lt of 43.3 ± 14.4 years. rates of recipient mortality, graft failure, biliary complications, acute cellular rejection, and reoperation were similar in both groups. postoperative cholangiography was used more frequently in patients with dd reconstruction (33.3% vs. 8.6%, p = 0.026). objectives the aim of this study was to assess the safety and efficacy of dd anastomosis, compared to ryh reconstruction, among adults transplanted for psc.

Duct-to-Duct Biliary Anastomosis Yields Similar Outcomes to Roux-en-Y Hepaticojejunostomy in Liver Transplantation for Primary Sclerosing Cholangitis
متن کاملDuct-to-duct biliary reconstruction in liver transplantation for primary sclerosing cholangitis is associated with less biliary complications, compared with Roux-en-Y hepatico-jejunostomy
Background There is no consensus on the preferred type of biliary reconstruction in patients undergoing orthotopic liver transplantation (OLT) for primary sclerosing cholangitis (PSC). Aim of this study was to compare long-term outcome after OLT for PSC using either duct-to-duct anastomosis or Roux-en-Y hepatico-jejunostomy for biliary reconstruction. Methods In a consecutive series of 98 adult...

background bile duct injuries (bdi) during cholecystectomy are now being recognized as major health problems. objectives herein, we present our experience with handling major bdis and report long-term outcome of hepaticojejunostomies followed by trans-jejunal hepatic duct stenting performed to reconstruct extra-hepatic biliary tracts. materials and methods in this case series, we prospectively ...

RATIONALE Choledocholithiasis in common bile duct (CBD) stump after Roux-en-Y hepaticojejunostomy (RYHJ) is incredibly rare and its pathophysiology is poorly understood. PATIENT CONCERNS A 79-year-old woman was admitted to our hospital with upper abdominal pain radiating through to the back in November 2016. DIAGNOSES Abdominal computed tomography (CT) scan and magnetic resonance cholangiop...
متن کاملThe bile duct anastomosis in liver transplantation.
In spite of the dramatic improvement of survival after orthotopic liver transplantation over the last decades, biliary tract complications are still the 'achilles heal' of liver transplantation with a potential risk of significant morbidity and mortality. Biliary leaks or strictures, the major types of biliary complications, may occur after direct choledocho-choledochostomy (CC) or Roux-Y chole...
